Ask about the exact prescription
Coverage depends on the product, plan rules and clinical circumstances. A service saying it works with insurance is not a promise that a particular medication will be covered.
Find the owner of the paperwork
Before enrollment, ask who submits prior authorization, who responds to requests for information and how you will learn the outcome. Establish the process for an appeal if one is appropriate.
Keep paying-for-care separate
A membership can continue while a coverage decision is pending. Ask whether the service pauses, refunds or continues charges during that period. Request the answer in writing.
Have a fallback budget
If coverage is denied, request a fresh cash quote rather than assuming the covered estimate still applies. Discuss any clinical alternatives with the prescriber.
