Semaglutide for fatty liver: ESSENCE and MASH

ESSENCE measured liver-biopsy changes in MASH with significant fibrosis. “Fatty liver” is a broader term than the population and indication.

The answer: improvement in two biopsy endpoints

In the first 800 participants assessed at 72 weeks in ESSENCE, MASH resolved without worsening fibrosis in 62.9% receiving semaglutide versus 34.3% receiving placebo. Fibrosis improved without worsening MASH in 36.8% versus 22.4%. Participants had biopsy-confirmed MASH with stage F2 or F3 fibrosis.[1]

These are two separate endpoints. The first asks whether the inflammatory liver disease resolves while scarring does not get worse. The second asks whether scarring improves while the inflammatory disease does not get worse. Meeting either definition is more specific than a change in a blood test or the phrase “better liver health.”

Fat, inflammation and fibrosis are different measurements

Three layers of a liver-health headline
  1. SteatosisFat accumulation
  2. SteatohepatitisFat-associated inflammation and injury
  3. FibrosisScarring, graded by stage

A person told they have fatty liver does not automatically have the same condition as an ESSENCE participant. The presence of fat, inflammatory disease and the extent of scarring are related but distinct clinical questions. That distinction is central to understanding which evidence applies, and why a generic wellness panel cannot stand in for a disease assessment.

EndpointSemaglutidePlacebo
MASH resolution without fibrosis worsening62.9%34.3%
Fibrosis improvement without MASH worsening36.8%22.4%

The table uses proportions of participants meeting a definition, not average percentage reductions in liver fat or scar tissue. Saying “fibrosis fell 36.8%” would be incorrect. The number means that 36.8% met the trial’s improvement endpoint. It does not describe how much tissue changed in each responder.

What accelerated approval does and does not mean

Wegovy injection has an accelerated-approval indication for adults with noncirrhotic MASH and moderate-to-advanced fibrosis consistent with F2–F3. The label identifies improvement in MASH and fibrosis as the basis and states that continued approval may depend on verification of clinical benefit.[2]

This distinction connects a measured intermediate outcome with the longer-term outcomes patients ultimately care about. A favorable biopsy can be meaningful without already proving fewer cases of liver failure, transplantation or death. The approval pathway allows earlier access while the clinical-benefit question continues to be evaluated. It should not be described either as no approval or as completed proof of every long-term liver outcome.

Why the product and disease stage stay attached to the result

An approved injection studied in this population is not interchangeable evidence for a compounded dissolvable tablet or an unspecified “liver peptide” program. Changing the preparation changes the evidence question. Changing the population from F2–F3 noncirrhotic MASH to all people with fat on an ultrasound changes it again.

Disease-specific care also needs a plan for reassessment. Weight is easy to measure at home, but it does not tell a patient whether fibrosis has changed. A care plan should explain which clinical measurements will be followed and how the treating team will interpret them. This is a different service requirement from receiving a prescription and tracking weekly pounds.

Where to go next

Read our oral formulation article before transferring injection results to another preparation. The Ro review and coverage-support comparisons discuss access work, while the study-reader checklist helps record the actual diagnosis and endpoint. A provider’s weight-management offer should not be assumed to include specialist liver assessment; confirm the scope of care separately.
