Collect the records first
Ask your current care team how to obtain your prescription history, relevant visit notes and pharmacy details. The next clinician may need a fresh assessment and does not have to adopt the previous plan.
Know which charges continue
Find the old service’s cancellation deadline, remaining term and final renewal date. Starting another service does not cancel the first subscription.
Confirm responsibility
Ask which clinician will handle questions during the transition and when the new service can actually begin care. Discuss treatment continuity with a clinician rather than adjusting anything yourself.
Close the loop
Save written cancellation confirmation, obtain any records you need and verify the new service’s full quote. A smooth handoff means knowing both who provides care and who is still billing.
