Start with five separate lines
Record the enrollment fee, recurring care membership, medication, required testing and shipping. Mark a missing price “unknown.” An undisclosed medication charge is never zero.
Separate today from later
Introductory offers can expire; prepaid plans may collect several months at once. Write down the payment due today, the next renewal amount and the length of the obligation.
Four weeks is not a calendar month
A four-week billing cycle is 28 days. Thirteen cycles cover 364 days. A calendar-month plan normally has twelve payments per year. Compare the total over a common period, not just the label.
Insurance estimates need a second column
Keep the hoped-for covered amount separate from the confirmed pharmacy quote. Ask about prior authorization, exclusions and what the care membership costs while coverage is unresolved.
